Metro Home Health Care is committed to making sure every person we serve receives the direct care and support they need. As our organization continues to grow, part of our commitment to client choice, we maintain a coordinated partnership with two independently owned and operated agencies Minnesota Home Care Provider and Platinum Home Care, to help expand access to services across Minnesota. This partnership is especially valuable for individuals under age 65 who may need additional support options.
COVERAGE AND ELIGIBILITY
Each agency operates under its own licensure, staffing, and insurance contracts. Because Minnesota Home Care Provider and Platinum Home Care are contracted specifically with Medical Assistance (MA), they are often well-suited to serve individuals under age 65 whose coverage is MA-based. Metro Home Health Care, which holds contracts across a broader range of insurance plans, continues to serve clients of all ages whose coverage fits our contracts.
In all cases, the choice of provider remains with the client or their legal representative. If you apply through Metro Home Health Care and your coverage is better matched to one of our partner agencies, we'll help guide you to the right fit while keeping you fully informed about which company will be providing your care.
PARTNERING WITH
ABC CHORE SERVICES FOR A SAFE, CLEAN HOME
We also partner with ABC Chore Services LLC to help clients maintain a safe, clean, and well-kept home environment. Chore services can include lawn care and seasonal yard maintenance, snow removal, and dumpster rentals for home clean-outs and decluttering projects.
SERVICES AVAILABLE THROUGH ABC CHORE SERVICES
ABC Chore Services LLC provides dependable household and outdoor maintenance services for eligible individuals who need assistance maintaining a safe, clean, and healthy home environment.
We accept HealthPartners Elderly Waiver (EW) referrals and authorizations. We are also contracted to provide chore services for eligible participants in the following Minnesota Home and Community-Based Services (HCBS) programs:

Services are available to eligible individuals through participating health plans, counties, and case managers. If you are enrolled in one of these programs and need assistance with household or outdoor maintenance, please contact us or speak with your case manager to determine your eligibility and begin services.

CFSS IS NOT REPLACING
YOUR CARE (PCA)
IT'S EXPANDING YOUR CHOICES AND METRO HOME HEALTH CARE WILL BE WITH YOU EVERY STEP OF THE WAY.
Minnesota DHS is transitioning the Personal Care Assistance (PCA) program to Community First Services and Supports (CFSS). A more flexible, person-centered program that gives you greater choice and control over your care. The transition runs through September 2026.

PCA CLIENTS
Your services will not be interrupted during the transition. Your case manager will contact you before your annual reassessment to explain your CFSS options, including the possibility of paying a family caregiver.
